Decision quality, key-person risk, and succession planning may share a partly neglected input: the capacity of the people making the highest-stakes calls. Bio-Optimized Leadership proposes Biological Capacity as a latent, multidimensional construct. That proposition is not yet a validated leadership KPI.
Most organizations measure business performance, talent movement, and engagement but do not measure whether capacity indicators add useful information about leadership continuity or performance. The relevant question is empirical: do well-specified measures improve explanation or prediction beyond simpler alternatives?
The evidence is substantive but bounded. A meta-analysis covering 70 articles, 147 cognitive datasets, and 1,533 healthy adults found that 24–48 hours without sleep had its largest effects on lapses and slowing in simple attention, while reasoning accuracy showed a small, statistically non-significant change (Lim & Dinges, 2010). Among 156 Swedish CEOs, higher self-reported burnout was associated with lower firm EBITA obtained from official records, but the observational design does not establish causation (Sirén et al., 2018). Swedish register evidence also indicates that health predicted who became and remained CEO, while becoming CEO had no discernible effect on the role holder's health (Keloharju et al., 2023).
What changed is the range of capacity-relevant measurement. Biological age sits primarily within Physiological Capacity; sleep, recovery, and direct functional measures add different information. In U.S. POINTER, a two-year randomized trial of 2,111 adults aged 60–79 at elevated risk of cognitive decline, both groups addressed physical activity, cognitive activity, healthy diet, social engagement, and cardiovascular health monitoring. Global cognition improved in both groups. The structured, more intensive program improved by 0.029 standard deviations per year more than the active, self-guided program. This was an older, at-risk population, not a workplace study, and the functional importance and durability of the small difference remain uncertain (Baker et al., 2025).
A KPI is something an organization defines, measures, and governs. BOL's research question is whether well-specified Biological Capacity measures add useful explanatory or predictive value beyond simpler alternatives for the outcomes that matter. Until that is tested prospectively, Biological Capacity remains a leadership measurement proposition, not an established performance indicator.
